If you'd like to apply for design help from PBD, please fill out this online form.
What we will need from you:
- Copy of 501c3 exemption Letter
- Copy of Insurance Certificate coverage
- Detailed contact information
- A brief description of the services your agency provides
- A description of the project
- Confirmation that your board has approved this project
- Insurance coverage details
- Can your agency provide any funding,trucking or volunteers?
- Suggested start date
- Suggested finish date
- The type of furnishings you need

Important, please note:
- You must submit proof of your agency's
501c3 in order to apply.
- You may have to make your own arrangements
for picking up donated items.
- PBD is not able to do electrical, plumbing, or structural work in a facility.
- Any portion of the work requiring special permits and/or licenses will be the responsibility of your agency.
